The essentials

Overnight lenses temporarily and carefully reshape the surface of the cornea. They do not cure myopia, but they can correct daytime vision and, in children, help slow its progression.

The outcome depends on the prescription, corneal shape, consistency of wear and individual response. Only an assessment can determine whether this solution is suitable.

How do overnight lenses work?

An orthokeratology lens is a rigid, oxygen-permeable lens whose geometry is calculated from measurements of each eye. Worn overnight, it acts on the distribution of the cornea’s surface cells. This optical change is slight, controlled and reversible.

The lens is removed in the morning. Its effect generally lasts throughout the day, then gradually decreases if wear is stopped. The lenses therefore need to be worn regularly, following the instructions provided.

What the research says

Studies conclude that, on average, orthokeratology slows eye elongation in children with myopia. Effectiveness varies from one child to another: it reduces expected progression but cannot guarantee that it will stop.

Indications

Who may benefit from this solution?

Orthokeratology may be considered for some children, teenagers and adults with myopia, with or without astigmatism, depending on the parameters of the eye. In children, it is often proposed as part of an overall progressive myopia management strategy.

The assessment includes checks of:

  • the prescription and how it is changing;
  • corneal shape and regularity using topography;
  • the condition of the ocular surface and eyelids;
  • the patient’s and family’s ability to follow the handling, hygiene and follow-up requirements.

Corneal disease, active inflammation, inadequate hygiene or certain handling difficulties may mean that another solution is more appropriate.

A multi-stage fitting process

  1. Initial assessment.Vision, refraction, cornea and ocular surface are measured, together with axial length when relevant for progressive myopia.
  2. Design and collection.The lenses are selected or calculated, then the patient learns insertion, removal, cleaning and safety rules.
  3. First check-up.An early examination checks vision, lens centration and the corneal response.
  4. Adjustments and follow-up.Several check-ups may be required. Follow-up then continues to monitor effectiveness, eye health and myopia progression.

Realistic expectations

What results can I expect?

Vision often improves after the first few nights, then gradually stabilises. Additional correction may be helpful initially or in certain situations. Night-time halos, fluctuating vision or awareness of the lens may occur, especially at the beginning of the fitting process.

In children, follow-up is not based solely on dioptres. Axial length helps monitor eye growth and assess the response to treatment over time.

Safety

Hygiene and warning signs

Overnight wear requires meticulous hygiene. Wash and dry your hands, use only the prescribed products, replace the case as instructed, and never allow the lenses, case or accessories to come into contact with tap water, shower water or swimming-pool water.

Remove the lens and seek advice promptly

if the eye is red and painful, if there is marked sensitivity to light, discharge or an unusual reduction in vision. Corneal infection is rare, but it can be serious and requires prompt treatment.

Never wear a damaged lens and do not alter wearing time yourself. Check-up appointments remain necessary even when everything feels fine.

Do overnight lenses correct myopia permanently?

No. Their effect on the cornea is temporary and reversible. Vision gradually returns to its initial state when lens wear is stopped.

Can a child handle the lenses independently?

This depends on their age and independence. The child and family learn together and must be able to follow the hygiene rules every day.

Can I swim while wearing them?

No. The lenses and their accessories must never be exposed to water. The lenses are removed on waking, before showering or swimming.
